Purpose: The purpose of this consent form is to allow The Lab for Excellence in Dance, Ltd. (hereinafter referred to as “ICE”) to publish certain biographical information, videos and/or photographs (collectively, the “Content”) provided by the above-named dancer (the “Dancer”) on ICE’s official website and affiliated digital platforms (collectively, the “Platforms”), which Content shall be used for the purpose of celebrating participating dancers, and sharing their artistic background and achievements as part of ICE’s educational and artistic mission.
Acknowledgement: I, the undersigned parent or legal guardian, hereby grant permission for the Dancer to submit the Content for publication by ICE on the Platforms. I understand that the Content, to the extent approved by ICE, at its sole discretion, will be published and publicly accessible and may be viewed, shared, or downloaded by others, and that ICE cannot control such third-party use of the Content once published. ICE reserves the right to edit, limit, or remove any Content at its sole discretion.
Release and Waiver: On behalf of myself, the Dancer, our family members, estate, heirs, executors, administrators, assigns, and personal representatives, I hereby expressly waive any claims for damages, and release and hold harmless ICE, its directors, officers, employees, volunteers, and agents from any and all claims, liabilities, or damages, arising from or relating to the publication, use, or display of the Content, including any and all use by third parties once the Content has been made public.
Withdrawal of Consent: I understand that I may withdraw this consent at any time by submitting a written request to ICE at Dance@icensemble.org. Upon receipt, ICE will remove any uploaded Content from the Platforms within a reasonable timeframe, though it cannot guarantee removal from third-party sites or social media where the Content may have been shared.
Public Availability of the Content: I acknowledge and confirm that I have been informed, and fully understand, that the Content will be internationally available on the internet through the Platforms. This information may appear in online search engine results and can be accessed by the general public. By signing below, I consent to this public visibility and accept, on behalf of the Dancer, the associated risks inherent to online publication. I also understand and agree that the Dancer may have ongoing access to their personal profile in order to update, edit, or add to the Content in the future. Any such updates will remain subject to ICE’s review and approval before publication. The same privacy, public availability, and waiver conditions described in this document will continue to apply to any future updates or modifications made by the Dancer to the Content.
Authority to Sign: By signing below, I certify that I am at least 18 years of age and that I am the custodial parent or legal guardian of the Dancer.
